Job Purpose: As a HOPE Team Outreach Specialist
at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions regarding reintegration into the community of people who are homeless.
Job Functions
Providing outreach services Conducting assessments and completing referralsAssisting clients with obtaining necessary documents to obtain housingNegotiating the criminal justice system and advocating for diversion when appropriateAdhering to contractual obligations Serving as acting representative to committees and community planningCompleting required trainings and participating in required meetingsMost importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
Position qualifications: Ability to receive and maintain a(n)
Level II Background clearance Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1High School Diploma is required. Bachelor's Degree is preferred. 1 to 2 years of experience working with targeted population may substitute for higher education.
