Florida Department of Health in Seminole County is looking for a
self-motivated, hardworking individual to fill our Health Service
Representative position.
PRIMARY DUTIES AND GLOBAL RESPONSIBILITIES
Under supervision of the Operations & Management Consultant MGR -
SES the incumbent in this position serves the HIV/AIDS Program to
provide a comprehensive system of care designed specifically for
people living with HIV in Seminole County. The system of care
includes primary medical care coordination and essential support
services for people who are uninsured or underinsured. Incumbent is
responsible for assessing needs, developing comprehensive plans to
meet identified needs, prioritizing services, and evaluating the
efficiency of the system of care. It is the aim of this case
management position to increase the numbers of persons living with
HIV/AIDS who engage and remain engaged in HIV Primary Care,
increase the number of persons who obtain insurance and increase
the number of persons maintain viral load suppression. This
includes helping clients with critical HIV medications and health
services.
Incumbent is responsible for initiating and coordinating HIV/AIDS
program daily activities at the county level. These activities will
focus on: Ryan White A and B. Scheduling appointments/office
visits, answering phone calls, track and maintaining eligibility of
clients enrolled in the program, completing client profile (medical
records), trouble shooting for Healthcare Providers and pharmacies,
plan and coordinate national awareness days, special events, health
fairs and meetings in collaboration with DOH-Seminole. This
position serves as a liaison between the Ryan White and HIV/AIDS
programs, youth and adult clients, organizations, businesses,
DOH-Clinic, private providers, and other local agencies. This is a
direct service position.
Enter all client encounters both face to face and by phone into the
Health Management System (HMS) and Provide Enterprise System for
tracking and reporting purposes. Submits required reports,
maintains assigned records, completes/returns all paperwork
pertaining to job duties accurately, legibly and in proper order.
This would also include timely submittal of timesheets, mileage,
and EARS. Facilitate Quality improvement activities. Actively
participate in team meetings or case conferences. Maintain
appropriate relationships with Key Point of Entry (KPOE) into the
healthcare system. In addition, this position will conduct outreach
activities for potential clients to promote the availability of
services. Incumbent will participate in health fairs, community
events and collaborate with providers among other activities.
Performs all other duties as required.
Required Knowledge, Skills, and Abilities: position: Knowledge of
terminology, principles and analytical techniques of interviewing,
data collection and analysis principles and procedures. Ability to
prepare reports, understand and apply applicable rules,
regulations, policies, and procedures relating to Ryan White, ADAP,
State and Local Specialty Care HIV/AIDS Programs, understand and
follow legislative policy direction as established in Federal and
Florida Statutes; deal with the public in a tactful and courteous
manner; work independently, plan, organize and coordinate work
assignments, communicate effectively. Knowledge of
committee/coalition development and maintaining partnerships, word
processing skills and computer applications that will enable
individual with oral presentations
Qualifications:
Minimum
+ Do you have a Ba chelor's degree on Social Work, Psychology,
Counseling, Public Health or related (Social Science, Human
Services, Sociology)?
+ This position may be required to work before, during and/or
beyond normal work hours or days in the event of an emergency.
Emergency duty required includes working in special needs or Red
Cross shelters, or performing other emergency duties including, but
not limited to, responses to or threats involving any disasters or
threat of disaster, man-made or natural. This position will
maintain confidentiality and security of records as specified by
departmental regulations and laws.
Preferred
+ Do you have verifiable work experience working in a
multidisciplinary team addressing the needs of these clients?
+ Do you have at least 1 years of customer service experience?
+ Do you have at least 1 years of direct case management
experience?
