This position is for clinical services in the Family Planning, Sexually Transmitted Diseases (STD) and HIV programs and School Physicals and Immunizations with the following skillsets.

This is highly independent and responsible Advanced Practice Registered Nurse position with Clinical Services providing care to Family Planning, STD and HIV clients and School Physicals and Immunizations. This position is part of a multidisciplinary team that ensures proper care and treatment to clients seeking services in these programs.

HIV: Performs complete physical appraisal of HIV patients, appropriate to background and training. Compiles and evaluates medical histories and physical findings. Selects and uses diagnostic tests and procedures. Initiates diagnostic laboratory tests; interpreting and evaluating laboratory results. Monitors medication and initiates appropriate therapies consulting with SCHD physicians and infectious diseases specialists as needed altering medications according to variation in patient's response within established protocols. Provides timely referral for specialized care as needed.

STD: Performs STD exams, evaluations and management as deemed appropriate. Performs Family planning exams-including history and physical, complete assessment and plan. Provide variety of birth control options to those who medically qualify.

School Physicals: Performs school physicals for all ages needing school entry. These are simple routine physicals. Those needing specialized care will be referred out after consultation with MD. Participates in the orientation and staff development programs for personnel assisting in patient care identifying improvements and adjustments needed to ensure program effectiveness and efficiency. Provides technical assistance and guidance to the program nursing staff through comprehensive review of charts, records, referrals, etc. with a primary focus on patient outcomes and quality care. Identify alterations needed, implement corrective actions as required.

Family Planning: Perform contraceptives, male and female sterilization services, client education and counseling and documentation of services in the HMS Electronic Health Record. Also ensures review of laboratory results by physicians followed by timely case management, follow-up treatment and referral as indicated for the client.

Immunizations: Perform the review of Immunizations records for missing doses, administering required immunizations and preparing and issuing necessary forms for school, college or travel.

Administers immunization services in the clinic setting of the DOH-Seminole including satellite clinic sites utilizing the current CDC immunization schedule and utilizing safe recommended techniques for vaccine administration. Provides services to clients requesting immunizations to meet school requirements, occupational exposure requirements i.e., Hepatitis B, other adult immunizations, and flu vaccines during the Flu season. Participates in health fairs and immunization events in the community. Completes required documentation in Florida Shots and the Health Management System (HMS). Is a resource regarding immunization requirements and is able to assist with orientation and training for new nursing and clerical staff to the immunization program. Is knowledgeable regarding foreign travel vaccines, cost of vaccine and is able to research and advise clients on required and recommended vaccinations for travel to foreign countries.

Participates in quality assurance activities for the Immunization Program to ensure Childhood Immunizations levels meet or exceed State and National goals. Conducts daily vaccine inventory to ensure the highest vaccine accountability. Ensures vaccine viability through safe handling and storage.

Charting & Administrative Duties: Enters all client encounters - both face to face and by phone into the Health Management System (HMS) for tracking and reporting purposes. Submits required reports, maintains assigned records, completes/returns all paperwork pertaining to job duties accurately, legibly and in proper order. This would also include timely submittal of timesheets, mileage, and EARS.

Other Clinic Duties: Reviews medical records for clients who need rabies post-exposure prophylaxis and administers rabies post-exposure prophylaxis as needed. Participates in community outreach and education for high-risk populations and healthcare workers regarding TB and hepatitis A, B and C. Participates in quality assurance, quality/process improvement and innovation lab initiatives. Completes all employee activity records, timesheets and mandatory trainings on time as defined in performance plan. Cross-trains with staff from other departments and mentors' students as assigned.

Quality Assurance and Improvement: Assist with chart reviews and other QA/QI activities to improve clinic functions and to meet DOH-Seminole's mission statement and goals. Assess client's well-being including but not limited: medication adherence, reasons for missed appointments, housing conditions, transportation. Provide and implement clinical recommendations. Reviews and authorizes prescription requests made by pharmaceutical providers and clients according to treatment needed by each client. Provides education to clients, partners, team members and community. Identifies areas for quality improvement in clinical functions and leads and participates in quality improvement and innovation lab projects aimed at improving clinic performance and patient outcomes.

Outreaches & Meetings: Conduct outreach activities for potential clients to promote the availability of services and infectious disease education. Incumbent will participate in health fairs, community events and collaborate with providers among other activities. Actively participate in staff meetings and case management meetings for clients to improve assessment and to sustain retention in care. Participates in at least one agency workgroup or committee.

Other: Completes other duties as assigned. This position may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disasters or threat of disaster, man-made or natural. This position will maintain confidentiality and security of records as specified by departmental regulations and laws.

Performs all other duties as required, including emergency response situations.

Required Knowledge, Skills, and Abilities:

Knowledge of medical terminology, principles and analytical techniques of interviewing; data collection and analysis principles and procedures. Knowledge of advanced nursing principles including health assessment, physical exam and medication prescription. Ability to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to TB, Refugee Health and other programs as assigned, Ability to understand and follow legislative policy direction as established in Federal and Florida Statutes; Ability to provide customer service in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others; Skilled in critical thinking and problem solving techniques; Computer skills including word processing, data entry and sending and reviewing emails.

Qualifications:

Minimum -

* Licensed from the Florida Board of Nursing APRN in accordance to with F.S. 464 or eligible to practice nursing in accordance with F.A.C. 210-6.27
* Florida Department of Health is required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties including but not limited to, response to or threats involving any disaster or threat of disaster man-made or natural
