Description SHIFT: Days (rotating weekends)SCHEDULE: Full-timeCase Manager (Registered Nurse)SCHEDULE: - Full-time, Days (with weekend rotation)$7,500 sign-on bonus -available for eligible new hires!POSITION SUMMARY: - The RN Case Management Care Coordinator provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. - Enhances the quality of patient management and satisfaction to promote continuity of care and cost effectiveness through the integration of case management, utilization review management and discharge planning.QUALIFICATIONS:

• Registered Nurse (RN) -diploma program or completion of related health care program required
• Minimum of three years recent acute clinical practice or related health care experience required
• Florida RN license required; or appropriate compact license; or ARNPSKILLS, KNOWLEDGE & ABILITIES:Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills, strong knowledge of governmental and private organization and community resources.ABOUT US:Central Florida Regional Hospital is now HCA Florida Lake Monroe Hospital.
