Unlock your potential!Job Summary and QualificationsThe medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.
  Schedule: Full-time, Days (with weekend rotation)What qualifications you will need:
  • Master of Social Work (MSW) required
  • State licensure preferred
  • Certification in social work or case management preferred
  • Acute care social work experience preferred
  • Knowledgeable about internal, community and post-acute care resources
  • Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis interventionThis role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).HCA Florida Lake Monroe Hospital is a 230-bed acute care hospital. We serve the communities of Seminole and West Volusia County. At HCA Healthcare, we are committed to the care and improvement of human life. We are a Level II Trauma Center. We provide the only full-service cardiovascular program. Our program offers open-heart surgery and interventional cardiology. Electrophysiology, cardiac rehabilitation, and comprehensive diagnostic services are also offered by this program. Other services that we provide include orthopedic and spine care, as well as robotic surgery. We offer acute inpatient medical rehabilitation and complete imaging services.
